Job Category: NURSING & NURSING MANAGEMENT Department: NURSING ON-CALL Job Title: COMMUNITY HEALTH NURSE (PD) Job Summary: Summary: To fulfill the needs of patients in a variety of age groups, their family members, and the community by performing varied patient care tasks and functions in accordance with licensure, certification, training, and regulatory requirements. To respond effectively to new and changing working environments. To bear the responsibility and accountability for the nursing care that the patient receives while receiving services from Valley Home Care, Inc.
Education: Graduate from an accredited School of Nursing. BSN preferred.
Experience: Computer and keyboard skills. Medical terminology required.
Special Skills: Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing). Current and valid CPR certification. Demonstrates effective interaction and communication (oral, writing, presenting) skills. Ability to work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and a changing work environment related to changing customer needs. Ability to work cooperatively within the health-system; with patients and family members; and with multidisciplinary team members (i.e., physicians, Rehab, etc.). Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations. Ability to use critical thinking and clinical reasoning skills to effectively problem-solve and deliver care.
Supervision Expected: Functional supervision of the Home Health Aide as required.
Supervision Received: Supervision from the Patient Care Manager and professional and/or technical input from the Nurse Clinician.
Working Conditions: Exposure to normal patient care area hazards, infections, puncture wounds, strains and sprains, blood-borne pathogens, HIV related illnesses, and varying noise levels. Requires lifting, pushing or pulling of up to 50-150 pounds occasionally with frequent lifting and carrying of up to 25 pounds; considerable standing, walking, sitting, reaching, filing, typing, and photocopying. Occasional stooping, kneeling, and crouching required. Constant talking and listening required.
